MIROW Patient Status Designations Guidelines -- Glossary

Active Individual Immunization Status at the Geographic Jurisdiction level is assigned to an individual whose residence within the Geographic Jurisdiction has been confirmed.

Active Patient Immunization Status at the Provider level is assigned to an individual who:

  • has received an immunization from a Provider, or
  • whom a health plan has identified as a Ptient of a Provider, or
  • a provider has identified as a Patient, or
  • other medical Information has identified as a Patient of a Provider

Geographic Jurisdiction is any defined collection of locations.

Geographic Jurisdiction Level of Accountability is assigned for any individual who has been identified as residing within the boundaries of the Geographic Jurisdiction.

Immunization Home is the practice (Provider) where the Patient receives immunization services. The Patient can be active with many Providers, but only one Provider will be considered as the Immunization Home. A Patient’s Immunization Home can be determined by parent/guardian election, last immunization from a Provider, or assignment by a health plan. The specifics for the assignment of the Immunization Home should be determined by each IIS.

Immunization Status is a ranking term used to describe tracking of immunization activity of an individual at two different levels of accountability: at the Provider level and at the Geographic Jurisdiction level.

Inactive - Lost to follow up Patient/Individual Immunization Status at the Provider level and at the Geographic Jurisdiction level is the same and is assigned when attempts to contact an individual have been documented but there is no documented response received, or there is inadequate contact information for the individual.

Inactive - MOGE Patient Immunization Status at the Provider level is assigned when one of the following has occurred:

  • a Patient has moved out of immediate area
  • a Patient has gone to another practice, or
  • a Patient has moved with no forwarding address.

Inactive – MOGE Individual Immunization Status at the Geographic Jurisdiction level is assigned when documentation exists that the individual no longer resides in the geographic jurisdiction.

Inactive – Permanently Patient/Individual Immunization Status at the Provider level and at the Geographic Jurisdiction level is the same and is assigned when an individual is deceased.

Inactive-Unspecified Patient Immunization Status is identified at the Provider level only and is assigned when a Provider has determined that a Patient is no longer active for immunization purposes but does not specify a reason.

Individual Immunization Status at the Geographic Jurisdiction level is:

  • Active,
  • Inactive - Permanently,
  • Inactive - MOGE,
  • Inactive - Lost to follow up, or
  • Unknown.

Legal entity is either an individual or an organization [e.g. the patient’s school or a provider].

Patient is an individual who has been registered by a Provider Organization.  An individual may be recognized as a Patient of a Provider when:

  • Assigned by a health plan
  • Given an immunization by a Provider
  • A Provider identifies the individual as a Patient
  • Their birth is reported by a Provider
  • Other medical information identifies the individual as a Patient.

Patient Immunization Status at the Provider level is:

  • Active,
  • Inactive - Permanently,
  • Inactive - MOGE,
  • Inactive - Lost to follow up,
  • Inactive,
  • Unspecified, or
  • Unknown.

Provider or Provider Organization is a health care organization that administers immunizations.

Provider Level Accountability is assigned when a Provider Organization registers its Patients. At the Provider level, a Patient may be active with more than one Provider and therefore more than one Provider may be accountable for a Patient.

Reminder Recall Notification (otherwise known as RR notification) is notification that reminds Legal Entities about a scheduled immunization or recalls them when an immunization is missed which require possible response from that Legal Entity.

Unknown Patient Immunization Status at the Provider level is assigned when a Patient has been made known to an IIS via an electronic interface without status being specified.

Unknown Individual Immunization Status at the Geographic Jurisdiction level is assigned when an individual is at least 7 years of age with no documented immunizations after his/her birth dose, OR is an individual for whom no contact or event (vaccination, change to the record, etc.) has been documented in his/her IIS record for 10 years.
